2023-2024 RHS ATHLETE REGISTRATION

 

WINTER/ SPRING ATHLETES  

(DOES NOT participate in a FALL 2023 RHS team sport)

1. Complete PIAA SECTIONS 1-5, signed by BOTH Guardian and Student Athlete (Click HERE to print)

2. Complete SECTION 6: SPORTS PHYSICAL with a medical provider (Example: MD, DO, PAC, CRNP, or SNP) This form must be dated AFTER June 1, 2023

3. Complete the Permission To Treat form (Click HERE to print)

4. Turn all forms into the RHS Athletic Training Office by Nov. 10

   1. ​Hand your forms directly into the Athletic Trainers (Kevin Schneider or Kaylee Scheirer) AFTER school. The Athletic Trainers will NOT be in their office until after 2pm daily on a regular school day. The Athletic Trainers’ office is located in the new Wellness Wing, next to Mr. Friel’s (Athletic Director) office.

   2. If the athletic trainers are not in their office, there will be a place in Mr. Friel’s office you may drop forms off in.

   3. Do NOT submit forms via email

5. Pay dues

 

FALL ATHLETES  

If a Student Athlete submitted paperwork for a FALL sport or played a FALL sport this school year
(2023-2024), that Student Athlete ONLY needs to submit Section 7 (the recertification by
parent/guardian form).

1. Complete PIAA Section 7 (Click HERE to print)

2. Complete the Permission To Treat form (Click HERE to print)

3. Turn all forms into the RHS Athletic Training Office by Nov. 10. See instructions above

4. Pay dues
